I would like to place on record my sincere thanks and appreciation for all that you have done for me during the past 15 months. In particular I value the amount of time and perseverance that you devote trying to find a diagnosis and great credit is also due to the skills of Dr Bellamy for his contribution to this. Of course I am also grateful for your surgical skills in carrying out my operation and all of the aftercare that you gave me.
